We are a CQC-registered 24-hour service in Bolton, Greater Manchester offering trauma-informed, neurodivergent-affirming care. With an in-house therapist, registered mental health nurse, and dedicated support team, we help adults rebuild independence and thrive.

Who We Support

We Can Support

  • Adults aged 18+ with severe and enduring mental health problems
  • Individuals requiring support to develop daily living skills
  • Those subject to community sections of the Mental Health Act
  • Individuals with a Social Worker or Care Coordinator
  • Those with up-to-date CPA documentation
  • Individuals willing to participate in a recovery program
  • Those with identified funding and personal income

We Cannot Support

  • Individuals in acute phase of illness with high risk
  • First episode of treatment
  • Primary diagnosis related to alcohol or substance misuse
  • Detained under the Mental Health Act
  • Client unwillingness to engage with support

Our Care Pathway

The pathway provides guidance from referral to the completion of therapeutic support.

Referral

We require the referrer to submit a referral form to help us understand the client's needs and determine if our service can effectively meet them.

Pre-Admissions Initial Baseline Assessment

We complete a person-centred, strength-based assessment to explore what matters to the individual, focusing on their abilities and the support that will help them thrive.

Formulation

Before the client moves in, a therapist develops a detailed formulation using pre-assessment information, clinical insight, and input from the social worker, family, carer, or friends. This is reviewed every 6 months.

Therapeutic Support Plan

Within six weeks of admission, we co-create a personalised Therapeutic Support Plan with the client and professionals involved. This is reviewed every 4 to 6 weeks.

Risk Assessment & Crisis Plan

A risk assessment and crisis plan is completed with the client and professionals, identifying triggers, warning signs, protective factors, and strategies to maintain safety. This is reviewed every 4 to 6 weeks.

Outcome Measures

We use trusted tools like the Camberwell Assessment of Need Short Appraisal Schedule (CANSAS) to measure progress. These are reviewed every 6 months with the client and professionals.

Celebrating Success

We recognise and celebrate the client's achievements, progress, and new skills. These moments may be shared with family, carers, or friends with consent.

End of Service Transition

Discharge readiness is agreed collaboratively, with the client at the centre. Outcome measures track progress from admission, indicating when 24-hour support is no longer required.

Discharge Summary

The therapist co-produces a discharge summary with the client, outlining progress, strengths, and ongoing support needs. This is shared with relevant professionals and family with consent.

Six Weeks Outreach Support

Clients are offered a 6-week outreach support package to facilitate their transition into their new environment and support independent living.
